Transitional care is a type of care service given to patients who move from a hospital setting to a home setting. This type of care is necessary to help patients fully recover from their illness or injury.
A care manager can perform roles varying from assisting in the discharge process from the hospital or rehabilitation facility to ensuring that the house will be comfortable, safe, and a good place to recover.
A caregiver can perform roles such as:
– Assist in exercise, mobility, and other physical activities of the patient
– Support and assist in wound care
– Help maintain hygiene
– Help in feeding and medication
– Offer other necessary personal care as requested by the patient or family of the patient
