Transitional care is a type of care service given to patients who move from a hospital setting to a home setting. This type of care is necessary to help patients fully recover from their illness or injury.

A care manager can perform roles varying from assisting in the discharge process from the hospital or rehabilitation facility to ensuring that the house will be comfortable, safe, and a good place to recover.
