Providing conscientious care for the elderly is always a group effort, here at ACNS, where every client enjoys the minimum benefit of a two-person team managing their entire care plan. Our Caregivers provide the day-to-day, in-person services requested — all-the-while being supervised by a behind-the-scenes Care Manager.
Care Managers
Think of Care Managers as coaches. They help to ensure that clients follow doctor’s orders — especially helpful if a client has memory difficulties or problems reading fine print. They supervise intake evaluations, visitation, staffing, admission to ACNS and oversee hospital/rehabilitation discharges.
Care Managers also assist with: scheduling, transportation and attendance of doctors’ appointments; facilitating communication between medical providers and family; monitoring adherence to medical instructions, including medication reminders; and with overseeing special dietary needs through strict shopping guidelines and proper food preparation.
Caregivers
